Tests and Consumables

    How do I obtain consumables for the SFL tests?

    Parkway Laboratory Services will provide all the consumables for the SFL.

    What information is required on the SFL Registration and Laboratory Referral Form in order for tests to be processed?

    Please provide us with the following information in order for tests to be processed:

    • Patient’s  Name, NRIC, Birthday and Address
    • Clinic Name
    • Doctor’s Name and MCR number
    • HCI Code
    What are Parkway Laboratory Services’ requirements in order for the Chronic Diseases test to be processed?

    Please provide us with the patient’s height, weight and blood pressure. We will only be able to process the patient’s specimen if the relevant information is provided.

    What are Parkway Laboratory Services’ requirements in order for the Cervical Cancer (Pap Smear) test to be processed?

    Please provide us with the patient’s Last Menstrual Period (LMP). We will only be able to process the patient’s specimen if the patient’s LMP is provided. If the patient has reached menopause, please state the estimated year of her LMP.

    How do I add tests that are not part of the SFL?

    For any tests that are not part of the ISP, please order your tests using the Parkway Laboratory Services Laboratory Request Form. The clinic will be billed for any tests ordered outside of the ISP.

    What are the specimens and the respective specimen storage conditions required?

    Fasting Venous Glucose and Lipids Panel:
    5mL yellow SST tube (1 qty) AND 6mL Grey fluoride tube (1 qty)
    **To be collected within the day of draw (stored in fridge further away from freezer compartment)

    Repeat Fasting Plasma Glucose Test:
    6mL Grey fluoride tube (1 qty)

    Oral Glucose Tolerance Test (0hr, 1hr)
    6mL Grey fluoride tube (2 qty)

    Pap Smear (ThinPrep test)
    Thin prep bottle with cytobrush
    **To be collected within the day of draw (stored at room temperature)
